Answer in English as completely and honestly as possible. You must write at least 10 sentences.

1) What grade did you earn 1st semester? Why? What/how can you improve OR continue to do so you are successful?

2) What specific goals do you have for the 4th six weeks? What specific steps will you do to get there? How can Mrs. Baker help?

3) On the remainder of the paper, create two columns. In the plus column, please tell me what works for you, what you enjoy about the class. In the delta column (a triangle), tell me about changes that could be made to help you learn better. Remember, learning better does not equal being lazy. Make sure it is a realistic recommendation if you want me to consider it.
